Over the 2020 Holiday season, The Population Health Innovation and Policy (PHIP) team, led by Dr. Alisahah Cole, decided to contribute their resources and encourage others in need. After doing some research, the group selected Feeding America, a network of food banks that supports 40 million Americans who are battling hunger. The monetary support given from the PHIP team aided in providing 1500 meals to families in need. Proud to make a difference.

Caregiving That Works
Care Coordination Program Manager Sally Grove and Clinical Administrator Robin Broms recently met with state Senator Monique Limón and the Home Alliance of Ventura County. It was one of several meetings with California legislators designed to align our Master Plan on Aging, with a focus on "Caregiving That Works" and "Healthcare Re-imagined."

Nurses Staff Honored
The Ambulatory Care Coordination team expanded on Nurses Week to honor all of its staff. Daily themed virtual events, such as this May 14 Zoom all-hands celebration, helped the Western Division markets bond, feel appreciated and have a little fun, too.

#WhiteCoats4BlackLives
Dignity Health Joins #WhiteCoats4BlackLives. On Friday, June 5, Dignity Health employees and medical group physicians joined health care workers across the country participating in White Coats for Black Lives, to show their support for racial equity and justice, and to call for an end to racism and discrimination.

Homeless Health Initiative
Under the Homeless Health Initiative and in partnership with community-based organizations across California, we have supported the delivery of meals and food supplies in communities experiencing food insecurity, supported the expansion of shelter services to ensure that even those who were homeless could shelter in place, and provided flexible funds to non-profit organizations to fill gaps in services within their community such as childcare or community education.

Serving Our Communities
Greater Sacramento’s Ambulatory Care Coordination department recently volunteered at the Sacramento Food Bank, which provides food and clothing to those in need, as well as educational opportunities and family support. The team was tasked with sorting pallets of donated goods, checking expiration dates and product quality. They appreciated the opportunity to work together outside of the office and give back to the community they serve.

Generating Smiles
Ceara Rae, ACN clinical performance consultant, generated big smiles at the primary care practice of Dr. Megan Jhaver. Through excellent, coordinated care, Dr. Jhaver earned a significant gainshare check for high quality, cost efficient care on ACN’s Next Generation ACO Medicare model and their Aetna value-based contract.

Family Traditions Potluck
Arizona Care Network recently hosted a “Family Traditions” pot luck, where staff brought dishes representing their heritage or favorites from their childhood. Favorites included Mexican shredded beef tacos, Chinese dumplings and Kentucky blackberry cobbler. Each dish displayed a description and why it is special to their family. The international buffet was fabulous, of course, but the event also helped ACN team members get to know one another on a deeper level.

Stuff the Backpack
ACN team members helped “Stuff the Backpack” for the greater Phoenix Back to School Clothing Drive in August. As a special treat, staff wrote encouraging notes that they tucked into each backpack so every kid would have a great start to their first day of school.

Gratitude Grams
Everyone likes to hear that they are special and appreciated. Arizona Care Network used Gratitude Grams to share special messages with their teammates. Here’s how it works: staff pick up postcards with a piece of candy attached and write a personal note of gratitude to a co-worker (or two, or more!) and drop it in a special mailbox. Members of the Employee Engagement Committee deliver them right before Thanksgiving. Our staff of 84 produced more than 800 Gratitude Grams and the number grows every year!
